Planning a Rehabilitation Programs specific for the injured person’s needs and goals
Sourcing appropriate service providers for the injured person and their needs
Ongoing liaison with the interdisciplinary team
Reporting back to the insurer
Generating service requests for updated recommended services throughout the plan period
Ongoing case management and welfare provision of the individual
Liaison with friends and family of the injured person
Providing all appropriate supports for the individual and their family based off of their medical needs
Advocating for the injured persons wants and needs
Providing suggestions to the care team based off of the goals put forward by the injured person and the outcomes aiming to be achieved
