What does our Case Management service look like?

  • Completing an initial assessment with the injured person

  • Planning a Rehabilitation Programs specific for the injured person’s needs and goals

  • Sourcing appropriate service providers for the injured person and their needs

  • Ongoing liaison with the interdisciplinary team

  • Reporting back to the insurer

  • Generating service requests for updated recommended services throughout the plan period

  • Ongoing case management and welfare provision of the individual

  • Liaison with friends and family of the injured person

  • Providing all appropriate supports for the individual and their family based off of their medical needs

  • Advocating for the injured persons wants and needs

  • Providing suggestions to the care team based off of the goals put forward by the injured person and the outcomes aiming to be achieved
